Appointments
Patients are seen in our office by appointment only. Please call our office at (203) 661-3333 to schedule an appointment with Dr. Ghogawala, Dr. Abbed, or Dr. Douglas. Please arrive fifteen minutes early for your first appointment to complete the necessary paperwork. Please download the
Patient Privacy Forms,
complete it and bring with you when you visit the office. If you are unable to keep this appointment, please call us at least 24 hours prior to your scheduled appointment time.
If you would like to submit your information online for requesting an appointment, please click on the

and hit the 'submit' button once you have completed the form. Our office
will get in touch with you the next business day. For all urgent matters,
please call our office directly.
For any medical emergency, please call 911 or proceed to the nearest emergency room.
If your insurance plan requires a referral from your primary care physician, please obtain a referral prior to your visit.

Financial Policy
Please call us for details about the insurance plans accepted by our office. We accept payments via checks, cash, Master Card and VISA. Please be prepared to make a co-payment at the time of your appointment. As a courtesy to our patients, we will submit your claim to your insurance carrier for both within network and out of network carriers.
Medication Prescription Refills
We will be happy to call in refills to your pharmacy for medications originally prescribed by us if you have an ongoing need for the medication. Please note the following:
- The physicians are generally in surgery on Mondays and Fridays. All prescription refills should be anticipated in advance so that they can be taken care of before the weekend.
- If you require a refill for a narcotic medication, such as Percocet and Oxycontin, you will need a written prescription from us. Pharmacies will not accept refill requests for narcotic medications over the phone.
- If you require an emergency refill when the office is closed, please visit the nearest emergency room.
